How to use this form: Fill in the information needed for us to make any busing changes you would like. Please note that these changes will be made in the order that they are received. No change will be made unless there is a valid phone number where a parent can be contacted. In order to assure the safety of your child, pickup and drop off locations are limited to one location each. In the event that you move or change daycare providers you will need to contact the bus garage at (906)786-4661 at least one week before the change will take effect. You will need to provide transportation for your child until busing can be arranged.
